Help / HELP! OnActivate is broken!

Every once and awhile, usually on my own servers, the OnActivate thing stops working. This means that I cannot open doors, press buttons, etc. OnActivate is really important, so if you know how to fix this problem, or if you have had the same problem, please try to help out. TY
