Help / I need help on a glitch

There is a glitch I have at my server. Every time i die it says that i didn't die,nothing pops up. But when someone else dies it says they die. And also when I kill someone in a minigame it shows the death CI and the players name and my name doesn't pop up at all. Please help tell me how to fix this glitch.

The Civil Protection Mask from Half-Life 2
NOTE: The thumbnail image is ment to be white,because the real mask in Half-Life 2 is white,But the face is transparent. P.S. srry i put it in the wrong section last time :P
