Has anybody ever made a scripting/modeling/positioning mistake where the weapon they have made turns out to face the wrong way in the players hand in-game?

An AA12 fully automatic shotgun. Possibly even a version that fires FRAG-12s, where the projectile hits a surface, and makes a small explosion radius.
