What would happen if a person set their status to busy but you had their icon set to some other icon ie: away, christmas, pumpkin...
Does it still show them as busy?

Yes
All it does is client-sided change the icons in your rtb gui

Cool, but useless.

I think it's pretty useful to "group" friends with icons and give special people a different one.

According to the script, Icon packs don't need a client.cs, description.txt nor namecheck.txt.

Put that in the command, and it works on other servers??...

No you put that directly in your console.
Also, (not sure) maybe you have to type clientcmdupdateprefs(); after it.
