For some reason, the lava wont damage players on my server. Is there any way to fix this?

Try going to Simple and switching to NONE then back to lava.

We did that and it still doesn't kill

Do you have any add-ons that could possibly conflict with this feature?

I think there is a setting that makes the lava kill or not.

I've already seen a lava in a server do an instant kill and at another server it slowly kills you.

If I remember correctly,,,, only the top layer of lava (the place between air and lava) hurts you.
