they used the item correctly, this was an unfortunate accident, no different then a car crash
If the swat knew how to check a house before going grenade happy this wouldn't have happened.
Just like how if a driver knew what he was doing on the road there wouldn't be a crash (with the exception of ice on the road or something similar that would forget them over)

Are you going to tell us next that an ar15 is strictly a military weapon and an assault rifle?  :cookieMonster:

ar15s are the semi automatic variants designed for civilians. So if your police are carrying those around thats pretty damn mild lol.
i know, im using that as a point that police carry more than a pistol and a baton

A child's crib being in the spot that the flash bang landed is ice on the road.

It's a combo of bad luck but also bad use of their equipment, and horrible intel/briefing to sum it all up.

I don't think I would go in and check the room if I knew there was a chance I would get a shotgun shell to the torso or to the head. Also a flashbang =/= grenade. Wasn't really the SWAT teams fault, it was people who told them to go into the wrong house and they were just following procedure.
It was just a freak accident.

It was just a bunch of bad luck with the throw, SWAT can't open the door to a group of gangsters, go "hi", throw a flashbang and close the door.

also, should probably realize, a flashbang isn't a grenade. it was meant to be non-lethal.
