If its hard to differentiate the humans form the virus, you should have the main character be someone that has the virus in a group of humans, and he has to fight back against his own virus, or do anything he has to do in secret, hoping they don't find out.

I suppose it's not really the virus, it's more what the vaccine has done to it and the patients. Instead of dying, they become angry mutants out to spread the virus, which now almost controls their thoughts and emotions. Almost like they are still people, but possessed by it as it was a demon or something.

But I still like the idea

What if the story is told from both perspectives, about a guy WITH the disease and the vaccine, and a guy trying to escape him. Should they have any relation?
