The "pictures" you're talking about are called decals and faces. To make one, take a transparent, 256x256 canvas in Paint.net or some other editing program and draw your face/decal. Save it as a PNG to your desktop. Resize it to be 64x64 and save it as the same name in a desktop folder named "thumbs". Make a ZIP folder called "Face_x" or "Decal_x" depending on what you made, replacing x with whatever name you want, like "Face_toxichazard" or "Decal_Armyshirts". Move the big picture and the thumbs folder into the new ZIP. Copy or cut the folder and place it in your Add-ons folder. Start the game, go to Player, and choose your decal/face. Note: Only people who have downloaded that face/decal will be able to see it. Otherwise they'll just see a default.

Get that, then right-click on your desktop and select "Create New"/"New" and "Winrar ZIP Archive".
