It's a bug years back that allowed people to upload avatars larger than 75x75 through other means. I remember back when it originally happened and it was fixed afterwards. I guess going back to the newer SMF broke them again?

Technically it was never fixed, although the exploit was mitigated by adding style="max-width:75px" to all avatars.
