When you spawn as a demo you hold the grenade launcher, but the sticky launcher is his primary, according to the loadout screen.

I've took out a medic in 1 hit using the ambassador, then backstabbed the heavy before he realised I'd killed his medic. All I'm saying is it's how you use the gun.
Basically it's how you use it that counts (don't you loving dare)

I want to destroy sentries - Use a demoman
I want to kill people - Use anything other then spy
i want to be a spy - Hide in a corner for 5 minutes waiting to kill a single engi (but then a demoman comes in and does it for you)

Both are primaries in a way.

You must have been in backstab range of the medic to get the heavy, unless he's dumb and not very attentive.
