He's saying how this game seems to mess with the whole planet/galaxy thing.
And since planets in this game can be ridiculously small, a collection of them is a galaxy.
And a collection of these "galaxies" are called "worlds"
And he's saying that Earth would be a "galaxy" and the Milky Way would be a "world" if our universe had Mario proportions

Man, those green stars are really well hidden. Especially the 2nd in Boulder Bowl Galaxy.
