3: Type +forward and +attack in console
4: Have your rocket launcher out
5: Take a daring adventure outside
Side note: Also if you want type mp_respawnwavetime __ whatever for faster respawning

I'm going to call my guess: New item builds level two equipment off the bat, but doesn't allow it to be upgraded any further. It might not even allow it to be repaired. The item might be the guitar.
