I always put the shotgun on my forehead and aimed the barrel away.. Is that bad?

"Any weapon + Forehead" will usually work too.

Although sneak can be very useful, I wouldn't use it too much, my first character was maxed sneak before he was 50 anything else and I would sneak through everywhere, eventually I got far enough in the story that sneaking wasn't good enough I couldn't kill the monsters because I had gotten almost no XP so I had to go back and grind through all the areas to get up some levels.
