I think it is milkshape itself, but try this.

For blender I use collision having a 0 LOD and everything else having 32.

Both of those have been tried to no effect.  Come on guys, it's not like he didn't do some intense searching.
Crysist:
Vehicle's everythings' LODs should be 16, except for the collision mesh, that should be -1.
