You're thinking about it too hard. This is TF2, where sandwiches and chocolate can be broken down into metal, and rockets are capable of health transfusion.

The whole point is that if you use the Sleeper, you can't one-shot heavier classes. You have to choose between killing enemies or supporting the team.
At least that's what I make of it.
