In Halo PC, when I got hit in the back with a Shotgun.

I watched Man vs. Wild.

Got in a fight with some goblins, one of them swung an axe at me and I dodged it, off a cliff. Fell about 10 z levels and shattered my lower spine after landing in a river then got eaten by sturgeon.
Dwarf Fortress adventure mode.
Rookie:
My arm was ripped off and I was then disemboweled by his foot.
