How did you last die?
Rugged:
Shoveled in Toribash.
Sivad:
Saints Row 2: I was acting all epic in singleplayer and I shot a RPG missile at a Ronin vehicle but when it exploded the hood flew off the vehicle and hit me.
Brickman:
A stromling pirate pushed me into a pit where I was smashed back into the bricks that I was built of.

Well I was on the final mission for ronins and I didn't know that I had to do strongholds to actually get more missions :/
