It's a choice between these as well, assuming you meet the criteria to access both
You have to beat the game (all the way through the Womb) ~10 times or not take any damage on the Womb final boss to be able to go to Sheol, and you have to own the Wrath of the Lamb DLC to be able to go to the Cathedral

So I was playing as Mr. Cyclopes man. Then I got number two. Then I got that single eye power up. Then I got tech 2. Then I got brimstone.
It resulted in an instant death laser that shot out slightly behind me and in front of me from some weird glitch that causes the laser to be off the normal starting point.
I killed moms heart in like 4 seconds but for some reason I didn't get to go to Sheol even though I didn't get touched.

yeah that happened to me also on my cain run :S

I think there is some kind of random dice roll that decides if the doors appears if you didn't get hit.

I'm getting kind of irritated that I'm still not getting the secret unlock notifications. I'm 4 items away from Platinum God and I really want to see that card that says I got it when it happens.

I think it actually operates like a Deal With the Devil room before you've beat the game the required number of times now that I think of it. That being that it's a random chance no matter what, but the likelihood increases drastically the less damage you take on the floor, with it being near 100% (if not equal to) if you don't get hit.
