The idea came from the game: Amnesia
It would be handy for a lot of kinds of servers, if possible with events: setsanity, addsanity, togglescary
The idea itself is that if you look to any brick/player/vehicle with that event on. your sanity will drain.
And without sanity your health will drain, And if possible that you gain sanity by seeing anything else for enough time.

i think he means something like an energy bar, only renamed as sanity and corresponds to light levels. when it depletes you get a bunch of black-out all over your screen.
