it doesn't do it for no reason
it's a user error that causes it
like moving the Xbox with a disc in it
as it clearly says not to do

windex is a glass cleaner; it will not get rid of scratches on a window, mirror, or disc
and that's the end of it

i heard smearing toothpaste on it with a soft piece of cloth helps but don't rub it in a circle around the disk, move the toothpaste to the edges, wash it with water, and then let it dry.
