There was an explosion on the launchpad recently. Richfrod, Jacov (he's prime minister AND an astronaut?!), and Billy-Bobry were in the command pod at the time of the explosion, and the rocket simply collapsed, luckily the crew was unharmed.
There was another accident while a rocket was in the ascent stage. Official reports list it as a structural failure, as the strap-on solid rocket motors providing additional thrust during the launch phase detached themselves and melted through the metal wall of a fuel tank, causing the fuel inside, and subsequently the rest of the rocket, to detonate. The command module survived, however the service module was blown to bits, raining debris across the space center. The same crew was aboard the rocket. Thanks to the structural integrity of the command pod, they lived to see another day.
