i would personally love to see a graphic update, fallout 3 looked way too brown, and the rocks looked identical to giant piles of stuff.
and maybe vehicles would be real cool
a bigger map would be great

Vehicles would kill the feeling of alone-ness you get from being in the wasteland.
Unless you had to scavenge all the parts for them and they broke fairly easily like you'd expect a jury rigged vehicle to break, I can see them being so abusable. In Borderlands, for example, I felt far too safe in vehicles. I just roadkilled everything.

That's what i mean.
Have a quest where you run from the starter town all over the map to find parts for it.
And it would be gaurded by tons of bandits and stuff.

I would want a realism mode which made everything that wasn't armored very well be incapacitated or die within a couple shots, just like in real life, and maybe give the player a tad bit more health than that.
