Sleek Black UI?

Hey I was going through some old BL pictures and I saw my old custom UI. Does anyone know where I can get it back?
Pics: Sorry If you cant really see but you know what I'm talking about.

The one called Burnt Glass is the best imo.

Once you download it, you'll need to use one of the methods for preventing the launcher from overwriting it, which I won't go into for lack of time.
