Actually the Imperium of Man, as a galaxy spanning empire, is surprisingly scarce on resources. There is also the issue that you could armor the soldier's faces of about a squad, which would be pointless as a heretic's bolt would tear through any flak armor, or you could bolster the armor of a tank, which is less pointless because tanks have very large guns. Which are suited for blasting through heretics, xeno, and deamon alike.

No, the reason they don't is because a ballistic mask is uncomfortable, limit vision, and cannot protect you from direct fire. And if even if it does stop the bullet you are still incapacitated because the force behind it broke your nose and/or blinded you. But go ahead and keep solidifying your spot in the 2014 handicap olympics.
