Name changes are very rare, I've only seen two or three.
I know Lugnut used to be Lugnut1026 or something.
Supreme Guy was once Supreme Commander, this can be backed up by one of his posts which says "Last edited by Supreme Commander". I forgot where it was.

And Ladios had his name capitalized, a post on the first page of the DOF screenshots says "Last edited by ladios"
