I always ctrl+alt+delete, and kill the task.

Same here. Cmd stopped responding once for me which led to an entire system failure. Turns out, when I got a jumpscare in Outlast, I kicked the tower and it loosend the PATA connector. It just happened to disconnect when I opened the cmd prompt.

Its sometimes from stuff you download. This happened while browsing XNXX which isn't much of a scammer sight.
