Broken Add-on help me

So, I started a fort wars and realized I had my DeathIonCannon Enabled. Which I uninstalled about a month ago. I Exited the server and went back to the add-on selection screen. I scrolled down to weapons and there was no DeathIonCannon. I then proceeded to by Add-ons folder in my documents and it wasn't there either. If someone could help me fix this isssue, That would be great.

It's failbinned.
I had the old crap oned fallout laser rifle it didn't appear so I looked in the console and saw its one of the CR'PED addons

I've had this problem before. But It depends on what type of machine you are using to host the server, I used a separate Windows 2007 server. When I restarted it, all addons were back to normal. It might be because you haven't restarted your computer in a month, or the server is reading from a different addons folder. Please elaborate more on how you are hosting.
