So this looks pretty good, also has good reviews
There are also achievements, and that's good cause I'm such an achievement hound

It's automaticly saved in a spreadsheet and I can delete duplicates. There were a couple of people who submitted 2 forms at the 10 random steam games.

Mostly because I already have all the games on that list except Blood Dragon which I'm not interested in soooooo...
