I think he said the the miniguns knockback combined with the jets can cause you to go up fast enough to kill yourself by hitting the ceiling.

And since there's no check for upward velocity, it shows the skull instead of the impact icon.

You must have some messed up add-on because in the default game it shows the regular impact icon.

The old M16 I had back in v8 did that and I hated but it was powerful as hell.
