Constant crashing on some servers

Sometimes when a server loads, it crashes for no apparent reason. The souce window says something about data, shapes, black or something along those lines. I have experienced this on at least 3 servers now. What is the problem, and how can I fix it?

This an official bug in BL, Badspot said it will be fixed in v13, it could be the host who has a defective add-on.
