There are a myriad of chromosomal syndromes the afflict the populace. For example Klinefelter's (XXY chromosomes) syndrome is present in 1 in 500 men, odds are that unless you've been living under a rock you've met dozens in your life and didn't even know it (they may not know either).
As far as shemales go... well they don't exist, sorry.
