Does EVERYTHING save?

On a Map does EVERY Single Last thing save on a map? because Im making a Medieval RPG and I'm going to have to build Stuff out of View so I can have different Islands Using Stuff like Set player transportation. SO the basic question is, is:

when you save does it save EVERYTHING on the map? Or only whats in view?

If it's out of range it cannot update that area

If you go out of range, you'll load what's there. If you come back where you can't see the out-of-range part anymore, it'll save what you last saw.
