Milkshape 3d help!!

i need help using milkshape 3d. i was going to design something but i dont know how to open the file to start. Do i need to download all three downloads or did i just open the wrong file?plz help if you can

there's alot of options but which option do i choose from to open and build with?just tell me the file in milkshape 3d that opens to make stuff plz.

i just downloaded all three from the download page the 3d studio max 6 download read me.txt says:Place max2dtloveporter6.dle in the plugins/folder?What is that?
