I can't remember, if a atom had one too many neutrons on its 3rd shell did that make it radioactive? Or was it radioactive if lost one neutron.

Black holes, decay.

Although technically when a blackhole "explodes" it releases a calculable amount of energy, so it isn't really lost.

Atoms are quite possibly made of even smaller things than those particles, smaller than even quarks.
