Minecraft screws up after I play it for over an hour.

So I decided to play Minecraft today, and usually after an hour or two the textures start to forget up (I sadly dont have any screenshots of it) but today, my items decided to forget up;

I literally have no idea how why this happens.
I also dont know where else I could put it, because I dont really wanna put this on the Minecraft Forums right now

very rarely on blockland some of the pixels around blocks will get re-aranged a bit

ok open up your windows start menu
in the search bar, type "run"
open up run
type in dxdiag
go to the display tab
find out what graphics card you have and google drivers for it
