on the Manage Subscriptions screen, which is the lowermost button on the panel on the left.

And if you go to http://www.youtube.com/feed/subscriptions you will only see uploads.

It only seems like half the website updates, going to subscription management brings me to an older style page.

You can set uploads only one by one from the main page as well. Just do it from the www.youtube.com/feed/subscriptions link.

pretty sure you can just skip the phone number

the first and last name are only used for a Google+ account that can easily be deleted later.

the email address is and was always used for account signups, so i don't see why you're complaining about that.

Yeah YouTube started using my full name, so I just deleted my Google+ account, keeping my Google account. Now my YouTube username went back to dkamm65.
