Probably the review process. If people just learned to make working add-ons they wouldn't have to deal with having their crap shot down by Mod Reviewers.

People who make good add-ons get them onto RTB and those are the only add-ons we care about.

I don't hate RTB. I just prefer to use my own hosting.
