When I make a model of something that has a part on your back (like the Med Gun, Should I separate the held item and the back mounted model? Or should they both be together. Ran into problems with the "both together" option being able to see the back mount in first person when looking down. Or is that just fixed with code?

The back in first person problem can be fixed with coding.

Isn't there something like detail level 999 when used in a model actually makes one thing show up in first person and the rest in third person? I read about it ages ago but lost the link, try looking for instructions on your DTS exporter.

Like how in Modern Warfare 2, your guy does thingsi n 3rd person like scratch himself, but you don't see him do it in first person?
