If you permaban, leave notes for the people that should stay banned no matter what. I perma ban someone until they ask to be unbanned on IRC, unless they are someone I noted with something that will remind me of how stupid they are. Don't give ban reasons like "Lol" or "Noob".

I keep the ban reasons simple. No spamming, Aimbot, No teamkilling, etc.
Sometimes I give second chances - but only when they actually ask for it.
