Email him very politely with a strong reason.

For example, my forum name used to be Gen. Nick. Same with my Blockland name but I eventually changed it to General Nick. I PMed him once to change it on the forums but he either never got it or was too busy and forgot. I PMed him again like a month ago to change it and he did.
