Basically, take the template (farther down) and put whatever image you want in the pic (don't forget the bottom text)
I've made some as well.
(my item is a test, and the other one was too big)

I cleaned it up a bit for you and modified the size so it was a bit more reasonable.

You had a bunch of scribbles up near the "?" image, and that itself was poorly cut and pasted on.

Copy paste it to C:\Windows\Fonts. It installs and will be used by all text programs.
