I got a perma-ban once, not exactly sure how, but it was right after retail came out. And because I didn't have any extra money laying around for retail at that time (or a good computer for that matter), I just decided to take a break with that ban. When I eventually decided to invest in Retail, I talked with Badspot about my ban and he was kind enough to unban me (It was at least a year since I was banned, and possibly some more time, before I was unbanned).
