Head's not too small, the baggy shirt makes shoulders look too big which provides an over-all air of weird proportions.

IMO the pants/skirt/whatever is positioned fine.

The shirt lacks a quality that would help me see that it was the bagginess. There are better ways to portray clothing. I'm no expert at it myself but I'm still under the impression that it's the shoulders at first glance.
