Then decide if she uses the key to escape or you do.

Nope, (spoilers)If you choose the head, she weighs it out and decides she should go and you should die. If you choose the heart, she gives her life for yours altruistically.

Well I haven't played it, I made guess by first observation (As in pausing game to look over notes.)
