I wonder how it keeps happening

Usually the animators are good about making sure they don't show, but the Flash objects for pony legs are single-sided, so sometimes a cutie mark will show up on the inside of the far leg's thigh.

Not being too savvy with how higher-end animation works in Flash myself I couldn't really tell you what that means, but that's how it was explained.
