The way I hold the mouse, the index finger is closer to the scroll wheel than the middle finger is. If I were to use my middle finger I'd have to shift my grasp on the mouse entirely, and that just feels weird.

I hold my mouse with my index finger on the left click, middle finger on the scroll wheel and my ring finger on the right click.

I use my Index finger for left-clicking, I use the middle finger for scrolling and clicking the scroll wheel, and if I need to right click, my ring finger moves to the right, and I use my middle finger to click that too.
