In menu: Click the Avatar button.
In game: Open the options menu and click the Avatar button in the upper right hand corner of the menu.
If neither of these buttons are there you may have an add on that is messing it up, trying removing client sided add ons from the add on folder and seeing if it works.

It does seem that you've had your avatar rights revoked. Go ask Badspot, it may be a mistake.
