It's loving hard to make scary things. This doesn't even seem disturbing. I made a list.

Dark holes.

Organic patterns and surfaces.

Kaleidoscope-patterns.

sharp lines, tendons, spines.

Inhumanly-large pupils.

Misshapen pupils.

High contrast.

High-number of teeth.

Very bright teeth, very dark mouth.

Disembodied eyes and mouths.

Things which resemble eyes.

Disembodied anything.

Nonono, pencil-tip sized pupils make it scary.

Take Majora's Mask for example.
The pupils were so small it made me sht brix.
