It appears that he has either killed his mother or found her dead, and the resulting mental illness is causing him to imagine receiving a phone call from her over and over again to pick up groceries. He's been to the store several times in a row, that's why there are already some bags on the table with the exact same contents and the cashier remarks that "we're in no danger of a milk shortage".

I have Schizophrenia, and it's not fun. The "voices" sometimes are recurring, and that explains the guy's cell phone speaking as his mother. The dead mother is a plot element, but the decapitation bit ruined it.

I don't know if this is important, but at the start the pattern of the groceries are different.
Anyways, to everyone who doesn't get it after reading everyones else:
The guy keeps hearing the cellphone call and keeps buying the milk and all that. And the cashier is saying they aren't running out of milk anytime soon. When he gets home to find his decapitated mother, he calls the police repeatedly so that one guy is talking about how the other one keeps calling 911.
