I've never seen one of these before, does he not put "permanent" in the bans now?

No. He did it to emphasize his hatred towards the person. A mere "ban" wouldn't be assumed to be permanent and thus wouldn't be taken seriously. Badspot has huge beef with people that ban for stupid reasons.

That was five years ago. I assume back then the format for notifying users of a ban wasn't the same.

It happened three days after the post. It's not long of a time.

Oh silly me I just saw 2007 to 2008 and for some reason thought the months were the other way around (Jan 2007 to Dec 2008)

Odd, I hope I didn't make too many mistakes like that on my finals the last few days :/
